Looked like he popped his collarbone to me.

After he stood up it looked like he was kind of holding or massaging his lower back. He's had lower back issues before. Telling the medics his back hurt would be reason enough for the c-collar. But if that was the case, he shouldn't have been walking. Should have been carried off.

His head took most the impact.  Looked like when he hit, he rolled as well and his neck got twisted.  

An impact to the forehead is the most common cause of fracture to the neck. Very similar to what happened to Dungey.
