I'm a size 12 and I used the 20mm. I had a KTM and I measured the distance from the edge of the peg to the back of the shifter tip and the Honda was 17mm shorter. That's why I went with a 20mm.

1/1/2024 12:16am Ohhhh on their website, and the sizes refer to shoe size? In thinking the 20mm extension but it says it’s for shoe size 14 plus and I’m a 12 which they offer 10mm extension which is only .3 inches. I just wanna make sure it fits right

You can also get 10mm further back footpegs as well... 5mm down also, if you are tall it helps.
