How about Bent Axle?, this looks way off...is your axle block deformed?

also>>>>> Maybe you installed your Axle block backwards from each other easy to do

Take the wheel off take the axle out of wheel...

Roll Axle on a flat surface to check for round out....

Put the axle in side with out wheel and match up lines on each side of the blocks,

take a level and see if you axle is plum of course your bike would have to be level where it sits.

Count from the back of the blocks on yamahas. The left block is missing two marks from the front.

Sight down the top of the chain, from the rear sprocket to the C/S sprocket. If the sprockets and chain are running in a straight formation, and the wheel is centered, screw what the marks are at. Mass production could have the marks off by a bit anyhow.

Skerby seems to have the answer to your problem ,though.

If the axle was bent, you would have probably never gotten it out of the wheel without some force.

Make sure wheel bearings are good.

The old school way of adjusting the chain is not going by the notches (marks) but use a tape measure from the swing arm pivot bolt to the rear axle on both sides.

I don't know about the Yamahas but could the one on the right be in backwards? Some have more meat on one side of the axle than the other. Make sure the front sides match.
