Transmission problems

My brothers 2012 Rmz250 will shift perfect 1-2-3 but when you try to go into 4th it will jump back down to 3rd then when you shift up again it will be in 5th.
It will stay in 4th if you don't give any throttle or if you only give it extremely light throttle. Like riding through pits to staging light.
We think it's either the dogs or the shift forks. The motor is pulled apart and there is some wear on the dogs in 4th gear but imo not enough to cause that problem. And the forks lol fine.
Seeing what answers we can get before ordering parts.

How does the shift drum / cam look ? All the channels still nice and defined, with no dingers ?

Drum looks very smooth. No obvious wear marks. We're mostly likely going to end up replacing th entire trans with a 2013 trans. Forks, gears, drum, and shift shaft. Only difference is the sprocket side is slightly bigger so need a new bearing for it and will need a 2013 sprocket to fit.
