YoT days I don't think it was A1 but more like San Diego Phil Alderton coyly showed me the holeshot devices on their factory Yamahas. I say factory because the first year the service program for the revolutionary 250F was the motors were only opened by Yamaha. There were enough motors for a rotation as I recall.
Someone find the earliest picture of these being used in supercross before 2003.. I can't find one.
Wanna say McGrath was using one in 2000-01?

It was definitely Yamaha that used them first in 1999 or 2000. They were using throttle tubes to make the part that went on the fork guard. Honda followed shortly thereafter.
An enduro guy and member of at least one ISDE team, Buck Walsworth from Valparaiso, Indiana made one for his Penton back in the early-mid 70s. Im not sure how it engaged, but from what I recall he used a clutch cable and an extra lever somewhere to release it. Buck was an early Penton guy and hung out with Gene Ritchie and some other midwest notables. I know this isn't a contemporary example, but I thought it was interesting. Im sure others played around with it over the years.
I believe it was early 2000s.. and those of us unskilled broke them early tryin to get the timing right when setting them! What a time to be alive!
