Definitely not reusable. Gonna need to be replated. And not sure what would cause your issue.

I would suggest you also look at the crank as it would seem that what ever caused this may reside there at this time. The cylinder can be fixed and replated.

The bottom notch was where the location pin was stuck to the cylinder and I pulled it off. 80% sure none of the piston got to the crank.

What brand piston? Check the exhaust bridge really close for cracks. I just went through this on my 02 cr125 ( same cyl ). The exhaust bridge can crack due to the power valve design. the pv design makes the bridge area very thin and pushes the water jacket away. Photo

The piston is oem, as about 6 hours on it

That’s not an oem piston. Ring stay is in wrong spot, wrist pin area looks totally diff, and that piston has markings on the top. This is a stock/oem piston Photo

Piston in pic looks like it may be a wiseco. I’ve herd people having issues with the numara ebay pistons simulate to what you had.

Yep, Namura is shit and the wrist pin clips are basically made of solder. Seen them fail personally a few times and the ones that stayed in almost fell out when I went to remove them. Never. buy. Namura.

Now would be a good time for an HPP cylinder swap though........
