It's funny that in the end credits the guy talks about how the pilot made a critical error by not making sure that he was attached. Seriously, his lack of responsibility amuses me, why wouldn't you make sure you were attached?

^^^ More than likely protocol for the company to make sure the client is properly secured. Maybe it was an oversight?
Same thing when you go parachuting. The company doesn't just say "Hey, put this on" hand you a parachute and expect you to properly put it on. They double check everything.
In this case maybe the guy thought he was securely hooked up seeing as he was wearing the harness.
Obviously only those that were there know the full story. Still amazing that he was able to hold on for as long as he did.
