I have never seen or heard of that ever anywhere else, im guessing either the groove for the clip was too deep, or the wrong wire dia , meaning it blew past it and then it fell off.
Anyway, it wasnt a clevis/ shock failure.
A few photos from the pits, final settings weren't dialled in until Saturday morning less than an hour before free practice started.... Gutted for the US team that it happened.

HRC with their thousand dollar foot pegs, perfect wire tying and dozens of clear protective stickers so nothing wears on the anodised parts... Seems like a real rookie error...
I like Seely but since roczens injury he's performed less stellar
Having measured literally hundreds (maybe even thousands) of circlips , and there associated grooves and mating parts , its possible that even a slight error in one , two or all 3 parts will give a failure.
I would expect that KYB use a standard clip, so a fault in the other two parts is possible, groove too deep in the clevis is my favourite.
Oh well...
