It could be an adjustable shoulder support that he can tighten as it gets fatigued for support

There is no telling what Euro doctors have come up with. I doubt it's a tens unit though. Unless it's a newer improved model. I'd imagine the arm pump from a tens would be unbearable. My wife wore an electrical stimulation unit after back surgery but it was very localized. Just a wag but some sort of relaxer...muscle or nerve?
